The Day Center of Evergreen seeks a Caregiver Support Coordinator

Location:        The Day Center of Evergreen
                    Located at 55 W 16th Street
                        
Work Schedule:    Full time – 32-36 hours per week
                         Work schedule typically is Monday through Friday from 9 am- 4 pm
                         Additional days may be scheduled per department needs, including some                            evenings
 

Summary:   In this role the Caregiver Support Coordinator will assess, support, guide, educate and address the needs of the Day Center participants and their caregivers.  In addition, this position will coordinate, evaluate, and expand the Caregiver Support program. The Caregiver Support Coordinator works closely with the Day Center team to insure that the needs of participants and their caregivers are being addressed. 

Qualifications:
•    Bachelor’s Degree in Social Work, Gerontology, Health or Social Services
•    Minimum of three (3) years of experience in the following areas:
o    Working with older adults, particularly those with dementia related diseases
o    Caregiver support
o    Program Development
o    Group Facilitation
o    Experience teaching/instructing as it relates to caregiver support and education
o    Community outreach
•    Prior case management experience a plus
•    Ability to work cooperatively in a team environment with a diverse group of clients,            caregivers, co-workers, volunteers, human services professionals and organizations
•    Excellent interpersonal and communication skills – oral, conversational, telephone and          written, bilingual Spanish/English a plus
•    Proven ability to maintain accurate records and prepare professional correspondence and      reports
•    Strong computer skills required: fluency with basic software such as Microsoft Office and      Outlook,  prefer experience with Vendor View and CareEvantage software or similar            database programs
•    Clean criminal and background record is required; Ability to pass pre-employment              testing and have reliable transportation

Responsibilities Include:
•    Connect with each caregiver associated with a participant upon admission of said                participant to the Day Center or on referral from other community sources
•    Provide follow-up support for each caregiver at a minimum of every three months and          complete caregiver assessments every six months.
•    Facilitate family care conferences and ongoing communications, acting as a liaison              between the participant, family and Day Center
•    Identify, suggest, and develop educational information, resources, tools and activities to        be implemented as part of a comprehensive caregiver support program, requiring contact      with the community, employers, staff, caregivers, respite providers and volunteers
•    Coordinate, plan and facilitate all caregiver support groups and caregiver training classes
•    Track and report all caregiver data necessary to meet the requirements of funders and          the needs of management and staff
•    Assist the Nurse and Program Manager with admissions when needed;  participate in            care conferences for each participant in the Day Center program
•    This position provides initial contact with prospective participants, conducts intake              assessments and interview clients, communicates client status/billing changes with              billing staff, and ensures that all admission/discharge paperwork is correctly filled out          and forwarded to the appropriate staff in a timely manner
•    Maintain a thorough understanding of all the programs and services provided and                supported by Care Services
•    Attend team meetings and community networking events as deemed appropriate
•    Supervisor/mentor social work/counseling interns
•    Other duties as assigned
