Adult Day Services Referrals
From professionals, including doctors,
discharge planners, rehab personnel, case managers...
Services provided in
our program
include, but are not limited to, the following:
- Nursing assessment and development and implementation
of a plan of care recognizing each persons unique strengths and needs
- Administration of medications as ordered and
monitoring therapeutic actions and/or adverse side affects
- Monitoring and intervention for unstable medical
conditions

- Training in self-administration of medication
- Training and education regarding health and wellness
- Consultation with primary care physician as
requested/needed
- Monitoring of vital signs and weight
- Monitoring blood sugars via glucoscan per order of
the physician
- Colostomy care and urinary catheter care as needed
- Participant and family linkage to other community
agencies regarding care issues
- Family and participant consultation regarding care
issues
- Individual/group therapeutic activity program
- A hot meal and nutritional snacks along with
supervision of modified and therapeutic diets or supplemental feedings
- Screening and implementation of a maintenance
exercise program in consultation with professionals from Holland Community Hospital
- Opportunities to socialize with peers, share feelings
regarding later life issues, and participate in field trips
