Case Coordination

Our trained, professional social workers provide in-home assessments, coordinate necessary services, and provide on-going monitoring for frail, at-risk seniors to ensure their well-being and safety.

Congregate Meal Program

This program offers lower income seniors, 60 or older, the daily Evergreen meal served in our dining room, with payment in the form of a meal voucher.

Caregiving Assessments, Support Groups, and Training

It's time to review your PART D Insurance!

Every year, Medicare Plans may make changes to their coverage, benefits, and costs, so every year Medicare participants should review their health and prescription coverage.

November 15 - December 31 is the only time you can enroll in or make changes to your Medicare Health and/or Prescription coverage for the next calendar year.

Call the Medicare/Medicaid Assistance Program at 800-803-7174 or Evergreen Commons at 616-355-5118. Well trained counselors at either location can access information for you and/or enroll you in the plan of your choice.
