Fitness Centers - Information

To use the Evergreen Fitness Center, or the Family Fitness Center at Evergreen rates, you should:

1. Join Evergreen Commons. Applications are available at both facilities or click here: Membership Application
2. Complete and return a physician release form and an enrollment form.  Both forms must be completed, dated, and signed as indicated. To print copies, click here: Fitness Forms
3. Attend Fitness Equipment Orientation. Sign up times are available.
4. Purchase a Fitness Plan or pay daily fees. Click here for options: Fitness Plan Options
5. Use your Scan Card when you arrive to use the facilities.
